The rubric

Every entry carries a severity score from 0 to 100, computed from a five-factor rubric - harm, scale, irreversibility, autonomy, and precedent - each scored 0-4 and combined with fixed weights (rubric v1). The combined score is never hand-assigned; it is always recomputed from the underlying factors, so a published score can never disagree with its own breakdown.

  • Minor (0-20)
  • Moderate (21-40)
  • Elevated (41-60)
  • Severe (61-80)
  • Critical (81-100)
